20Minutes in the Life of a Hatzolah Volunteer

By M.S.

9:03:00 PM "Attention all medics in the Rova: accidental gunshot wound at the army base at the corner of Rehov HaYehudim and Rechov Plugat Hakotel." The voice of Yossi Ben Shachar, the crew chief in the Old City of Jerusalem, crackles over the Hatzolah radio. Moshe Simons, working from his home on Rechov Ohr HaChaim, races from his computer, stopping only to put on his ID vest and to pick up his first aid kit containing bandages, IV's and resuscitation equipment, and his oxygen tank. Yehoshua Shetreet takes off like a rocket from his home in the Yeshivat HaKotel Kollel building, stopping at the entrance to pick up his first aid kit and oxygen canister. Tzvika Greenspan, a teacher at Yeshivat Netiv Aryeh, comes from his home on Rechov Bet El, and two additional EMT's - Ahra'le Bar-Lev and Dani Neiman race from Yeshivat HaKotel with first aid equipment and oxygen. In all, five EMT's are en route to the scene, each one unaware that the others were coming. There is no time to arrange who is coming and bringing which equipment - everyone just drops whatever he is doing and runs.

9:03:45 Tourists in the Rova stare in wonder at the orange vested men running at full speed in the direction of the army base. They try to ask them what's going on, but are left in the dust as the Hatzolah/MDA volunteers race to the building on foot.

9:04:15 All the EMT's arrive at the building simultaneously. Soldiers instructed to lead them to the scene in order to give treatment in the quickest possible manner meet them outside. In the meantime, Yossi Ben Shachar rushes to the Rova parking lot to bring Ambulance #70 - the Old City ambulance - to the scene.

9:04:30 Soldiers are hurriedly taking the five EMT's to the scene, but it seems that the accident occurred at the very top floor of the building - four stories up. Loaded down with oxygen and rescusitation kits, the EMT's make their way to the top floor.

9:05:15 Reaching the top floor, the Hatzolah/MDA members enter the room the soldier is in. He's lying on a stretcher, with a bullet wound on the left side of his chest. Blood is seeping out below him, and bloodstains are evident throughout the room. A massive tumult is going on - with a large number of unnecessary people milling about the room. Yehoshua immediately instructs all the superfluous people to leave, while the other EMT's assess the situation.

9:05:35 Yehoshua and Tzvika start treating the wound - preparing a Vaseline bandage used in cases of suspected pneumothorax. In the meantime, Moshe and Ahra'le open an oxygen canister and give oxygen to the patient, who is in obvious respiratory distress. Dani is in contact with Yossi, making sure that a mobile Intensive Care Unit is on the way from MDA.

9:06:30 The patient is holding steady with a pulse of 84, and Moshe listens to the patient's lungs, hearing clear chest sounds from his right lung. The left side of his chest sounds abnormal, with a bubbling sound clearly heard. The blood from the wound is seeping into a cavity in his chest, making it more difficult for him to breathe.

9:07:30 The bandage on the patient's chest is complete, and he is lifted slightly to check for an exit wound on his back. An exit wound is found near his shoulder, and the bandaging of that wound is initiated. In the meantime, Ahra'le neatly inserts an IV, increasing the fluids in the patient's bloodstream.

9:08:30 The patient starts fading out, both from the pain and presumably from the loss of blood. Tzvika keeps up a steady stream of chatter with the patient, to ensure that he retains consciousness.

9:09:00 Yossi arrives at the scene, and sees that everything is under control. He departs to try to find a quick evacuation route that will save time transporting the wounded patient out of the building.

9:10:00 Eli Beer, Hatzolah's coordinator arrives, along with the mobile ICU from MDA. The paramedic on duty receives a report from Dani and Yehoshua, and sees that everything is taken care of. He decides on rapid evacuation, stopping only to listen to the patient's chest and to make sure the patient is still conscious. The Vaseline bandage on the chest wound is exchanged for an Asherman Chest Seal, fitted with a one-way valve to ensure that no additional air is sucked into the chest.

9:11:00 Trouble evacuating the wounded. The steps leading out of the building are extremely narrow and winding, and the stretcher will not fit through in a horizontal position. The stretcher needs to be tilted into an almost upright position to fit through the steps.

9:13:00 Yossi finds a slightly shorter way out of the building that saves time and energy. A side door is exited, then a short flight of steps descended. The stretcher is hoisted onto the shoulders of the soldiers carrying it, and they begin to run toward the mobile Intensive Care Unit.

9:15:00 As the wounded soldier is being loaded onto the mobile ICU, Moshe goes ahead to clear the narrow road inside the Rova of any people or cars, instructing them to stay off until the ambulance passes.

9:16:00 The mobile ICU departs to Haddassah Ein Kerem, with the patient in serious but stable condition.

9:16:30 The Rova Hatzolah members allow themselves a brief smile as they pack up their gear, knowing that without the quick response, the soldier would be in a far more dire condition.

9:23:00 Everything is packed up - the men depart to their respective homes and Yeshivot.

Note: This is a true story, having occurring on the 16th of May 2004 in the Jewish Quarter of the Old City of Jerusalem. Any of the EMT's there can verify to the authenticity of this account.
